Required Forms

This is our resource page. You can find all required forms and applications here. If you are ready to register your kid for upcoming session, fill out the quick template. You need to have your phone number and email address handy to complete the template. Once you complete your contact information, you will get a call from our office regarding further information and application process.


If you are  all ready to apply, scroll down this page, download and print the Parent Initial Questionnaire, Photo and Comment Parental Consent Form, and Home Literacy Checklist, fill them and bring them with you when you visit the K-16 Literacy Center at UT Tyler on the day of your scheduled meeting.

Form 1.

Parent Initial Questionnaire. Fill the form online here.

This form is required for your kids to be accepted into the program. If you choose a word copy, please, download a copy and fill the form in its entirety. Don't forget to bring a copy to your meeting with K-16 Literacy member.

Photo and Comment Parental Consent Form. Click here.

This is a required form. We sometimes use photos and comments about your child for academic purposes. Please, download a copy, fill the form in its entirety and bring to the K-16 Office at UT Tyler.

Form 3. 

Reading Log. Click here

Once you are enrolled in the program, we require you to fill out a reading log. It's a weekly reading activity log. You can download and print as many copy as you want. Or you can get copies from K-16 Center. Don't forget to request some copies in your next visit!

Is your home literacy friendly? Checklist here

You are your child's first teacher. Your home is where your child will get his or her first experiences with books and reading. Download a copy of the checklist and bring with you for your next visit to K-16 Literacy Center.
